Travel Advice 

If you require vaccinations to travel, you must complete a travel assessment form which is available from Reception or by selecting the 'Travel Form' below. This must be completed and emailed to reception.bennfieldsurgery@nhs.net or handed in to Reception prior to your appointment with the Nurse.

You will need to book a 20-minute face to face appointment with the Nurse, ideally 4-6 weeks before travel. 

Practice Nurses will only administer recommended NHS funded vaccines and cannot provide payable vaccinations or anti-malarial medication. 

It is the responsibility of the patient to seek a private travel clinic if the Practice Nurse advises further payable vaccines are needed. These private clinics can be found on the internet, we are unable to provide you with details of private clinics.
