You can submit your Friends and Family Test by completing the online form below. The name and date of birth fields are not mandatory and therefore you can submit your response anonymously if you wish.

What is Friends and Family?
The NHS Friends and Family Test (FFT) was created to help service providers and commissioners understand whether patients are happy with the service provided, or where improvements are needed.
Here at Bennfield Surgery, we welcome and value your feedback.
The FFT is a voluntary questionnaire which can be submitted to the practice anonymously.
The results will be collected, and we will analyse them to see if any action is needed.
This does not replace any formal comments/complaints procedure in place. This is to give us an overall impression of how we are doing.
You can complete the questionnaire by:
- Completing and submitting the form above
- Printing a copy of the word document and brinking it to the surgery/pop it in the post box.
- Download the word document, complete electronically and email it to reception.bennfieldsurgery@nhs.net
- Picking up a paper copy from reception

Do we hold the correct contact details for you? If not, please contact Reception.
We are unable to book appointments via email. Please either phone the surgery or come in to the surgery to book your appointment.
Out of Hours
Outside of normal working hours, please contact the NHS out of hours service by phone on 111.
