New Patients
Our Practice Area is defined on this map. We can only accept patients who's main residential address is within our Practice Boundary.
When you apply to register with the practice, you will be asked to provide proof of identity, so please bring this with you when you come to register. This could be in the form of a valid NHS card, birth certificate, driving licence or passport. We will need to keep a photocopy of this until your registration has been approved by the local Primary Care Trust after which we will destroy the copy we hold at the practice.
Nowadays you register with a Practice rather than with an individual Doctor, though some NHS administration has not caught up with this change. You may find that your new NHS Card when you receive it, for example, has a named registered Doctor at the Practice.
This is not relevant at our practice anyway. You may choose to see any of the doctors or nurses for your appointments. You may find that the doctor of your choice may not be available for an appointment with you at short-notice. You may book some appointment slots up to six weeks in advance.
